Die Magnetresonanz verzeichnet gerade in letzter Zeit große Fortschritte im Bereich der Technik. Die MRT wird, neben der Computertomographie immer wichtiger im klinischen Alltag. Ganzkörper-Untersuchungen waren bisher der Computertomographie und der Nuklearmedizin vorbehalten. Neue Spulensysteme, wie zum Beispiel TIM (total imaging matrix, Siemens AG, Erlangen), kombiniert mit einer automatischen Tischverschiebetechnik machen es möglich, Untersuchungszeiten, je nach Indikation, von unter 30 Minuten zu erreichen. In manchen Fällen ist es nötig Kontrastmittel zu verwenden, wodurch sich die Untersuchungszeit verlängert. Ganzkörper-MRT wird in der Praxis bei Patienten mit familiär bedingten Risikofaktoren für Screening- Untersuchungen eingesetzt. Die Ganzkörper-MR-Angiographie wird bei Patienten mit Risikofaktoren für arterielle Erkrankungen durchgeführt. Mittels Ganzkörper-MRT können auch onkologische Patienten im Rahmen von Verlaufskontrollen untersucht werden, wobei für diese Patienten PET/CT (Positronen-Emissions-Tomographie kombiniert mit CT) sinnvoller erscheint. Seltener wird die Ganzkörper-MRT in der Forensik für virtuelle Autopsien eingesetzt, wobei aber derzeit die Ganzkörper-Computertomographie die Untersuchung der Wahl ist. Ein weiteres Gebiet, wo die Untersuchung eingesetzt wird, ist die Ganzkörper-Fettmessung mittels Ganzkörper-MRT, welche möglicherweise bei Sportlern eingesetzt werden kann. Der erste Teil meiner Diplomarbeit befasst sich mit physikalischen und technischen Gegebenheiten der Magnetresonanztomographie, besonders aber mit den technischen Voraussetzungen, die für die Ganzkörper-MRT nötig sind. Im zweiten Teil meiner Arbeit beschreibe ich die klinischen Anwendungsbereiche der Ganzkörper-MRT. Weiters beinhaltet dieser Teil die Vor- und Nachteile gegenüber anderen Ganzkörper-Untersuchungen wie PET/CT und CT. Der letzte Teil handelt von der praktischen Anwendung der Untersuchung. Ich habe dazu ein einwöchiges Praktikum in einer der größten und bekanntesten Kliniken Deutschlands, der Universitätsklinik Großhadern in München absolviert, wo ich die beobachtet habe wie die Untersuchung dort angewandt wird. *****Magnetic resonance imaging (MRI) has registered a fast technical progress lately. So far, MRI was a rarely examination in radiology in clinical practice. Up to now Whole-body examinations were reserved to CT and nuclear medicine. Whole-body MRI is an examination without the use of radiation. New coil-systems, like TIM (total imaging matrix, Siemens AG, Erlangen), coupled with automated table movement makes it possible to achieve times, of less 30 minutes depending upon the type of examination. In some cases contrast agents are required, which extends the examination time. Whole-body MRI can be used in practice for screening examinations for patients with a family history of arterial diseases. Predominantly the whole body MR Angiography (whole body MRA) is an MRI study of the blood vessels. Whole-body MRI can also facilitate the diagnostic control of oncological patients. But for these patients whole body PET-CT (positron emission tomography) or whole-body CT is more relevant. Rarely, this examination is used in forensic medicine for virtual autopsies. This scientific range, however, is dominated by Whole-body CT. Another special range is body fat measurement by whole body MRI, which can be used, for example, for sportsmen. The first part of my dissertation deals with physical and technical facts, especially about the technical equipment which is required for the examination. In the second part I describe the clinical possibilities of Whole body MRI. Furthermore, this part includes the pro and cons of the MRI in relation to other diagnostic whole-body examinations, like Whole-body PET/CT and CT. The last part is about the practical application of the examination. I made a practical course at the radiological department of the clinic Großhadern in Munich, which is one of the largest clinics in Germany. At the Institute of Magnetic resonance I observed the application of Whole-body MRI for one week.
